HEADQUARTERS 88TH. INFANTRY DIVISION UNITED STATE ARMY APO#88 AG 200.6 SUBJECT: Award of the Bronze Star Medal To : Sergeant John G. Hornak ############ , Company L 351st. Infantry RegimentDuring his combat experiences , Sergeant HORNAK displayed skill, loyalty ,and a devotion to duty, that placed well above the average in his duties ,as mortar gunner and mortar squad leader. During the battle during the battle in the Itri Valley, on 19 May 1944, Company L had broken through the enemy lines and the enemy was withdrawing under the cover of a heavy artillery barrage. Although he was painfully wounded , Sergeant HORNAK stayed at his mortar and placed his fire effectively, so that he destroyed a number of the enemy as well as a great deal of there equipment and hampered their movement so well that the rifle platoons of Company L were able to take a large number of prisoners. On 8 Feb. 1945 in the vicinity of Livergnano, Italy, one of the Company patrols penetrated the enemy position and ran into a overwhelming force of enemy solders. They radioed for help and Sergeant HORNAK's squad opened fire. He placed his mortar fire so effectively that the enemy was pinned down and our patrol was able to return to our lines without the loss of a single man, During the Po Valley offensive, Sergeant HORNAK continued his meritorious service, often times going far beyond his own normal duties. His conduct under fire is in keeping with the highest tradition of the United States Army.
